NTSB - JetBlue A320 / Learjet Proximity Incident
 
The NTSB has cited lack of procedures in JetBlue A320 and Learjet proximity incident (Flightglobal)

With simultaneous operations on converging runways, I would have assumed there would be procedures in place.
